[PATCH] glx: Replace broken GLX visual setup with a fixed "all" mode.

While this could be a red herring, I can no longer get full screen video 
with Flash.

It could be this or one of the other patches from the 16branch wiki page 
that did it....

I'll try and isolate the problem but I wont be able to do so very easily 
right now.
