Unless you want the driver polling the monitor and all the output 
devices all the time (as afaik only a few devices do interrupts on CRT 
dis/reconnect), there should be a way for the user to tell the X server 
that a new device has been attached or that one has been removed.

If xrandr isn't the right candidate for this, then xf86VidMode probably 
is.
