Another thing to try is enabling load detection for the secondary DAC.  It's
disabled by default as it's somewhat unreliable, especially on cards with
TV-out and VGA since they both share the DAC.  Add:
Option "TVDACLoadDetect" "TRUE"
to the device section of your xorg.conf
