The above line wants to tell you that the hsync range you specified in
xorg.conf doesn't match the hsync of your 1368x768 mode line *hinting
even stronger*, so you should fix that, according to

A simple way would be to just not specify such limits in xorg.conf and
let EDID do the work.
As LCDs usually like vertical refresh rates of 60 Hz, you also
shouldn't use a 75 Hz mode.
