Look for the file .../xkb/symbols/us (probably under /usr/share/xkb or
/etc/xkb).  In there you'll find a section named "USA - Programmer Dvorak".

Starting with a copy of that, create a new section (remember to give it
a new, unique name on the xkb_symbols line) and edit it to match the
layout shown on that web page.
