Did you happen to boot with the tv-out cable attached?  It's possible
something the bios sets up is messing up tv-out.  Can you try booting
up without the tv attached?  Also, the following options may help:

# force default standard to ntsc
Option "TVStandard" "ntsc"
# force tv to be detected as connected
Option "ForceTVOut" "true"
