new radeon memory map fixes (#3)

I give a try to http://gate.crashing.org/~benh/radeon-memmap-7.0-3.diff and http://gate.crashing.org/~benh/radeon-memmap-drm-4.diff. But it seems both doesn't work as expect, at last here.

I have Pegasos 2 machine with Radeon 9000 card (128/128/DVI/VIVO) and after
applying this patches I can start the xserver without any problems, but
starting glxgars for example cause winodo decorations to blink and moving
gears window cause a heavy corruption on desktop. After I exit from glxgears
everything is fine (No corruption at all)

When I switch to EXA glxgears seems to works fine but for example Chromium
(Game) cause similar symptoms as glxgears on XAA.

Previously I used http://gate.crashing.org/~benh/radeon-memmap.diff and it
works fine, and this is the last version that works. 

I was able to reproduce this problem on my friends x86 machine (AthlonXP
1700 + Radeon 9000 (128/64/DVI/TV)
