Updates spec for Xfixes 3.1: Hide/ShowCursor

The convention for XFixes is different from other extensions; additions
to the extension are marked with a new major version. Yes, this is
contradicted by the specification; I'm afraid I took the boilerplate
version number spec from another extension.

Minor version number changes will indicate adjustments or
backward-compatible changes in existing functionality.

This should be Version 4.0 of the XFixes protocol.

Btw -- I've got a preliminary spec for 'pixmap lost' events for XFixes
sitting here as proposed by Egbert; it hasn't been implemented or
reviewed, perhaps we can consider this for 5.0 of XFixes at some point.
