XFixes protocol spec updates to match implementation

Okay - I've gone ahead and committed (with a slight rewording of the
ExpandRegion description after an internal reviewer pointed out my
previous description was a bit confusing).   If anyone has further
corrections, let me know or git commit them yourself.
