Various missing COPYING files and licenses in xlibs module

The following directories don't have a COPYING file:

DMXExt
EvIEExt
ScrnSaverExt
XCalibrate
XCalibrateExt
XF86DGAExt
XF86MiscExt
XF86RushExt
XF86VMExt
XTrap
Xevie
Xinerama
Xp
XvMC
Xxf86dga
Xxf86misc
Xxf86rush
Xxf86vm
xkbui

Of those directories, in most cases the COPYING file could be generated
by grabbing the appropriate licenses from the tops of the source files.
 However, of the above, the following also contain some source files
with no licenses either (note that I only looked to see that at least
one source file had a license, not necessarily all of them):

XF86DGAExt
XF86MiscExt
XF86RushExt
Xinerama
XvMC
Xxf86dga
Xxf86misc
Xxf86rush

Several directories also had present-but-empty COPYING files (either
completely empty, or containing only a CVS keyword):

FS
RecordExt
ResourceExt
Xaw
Xfont
Xmu
Xrandr

Of those, none have missing file licenses.


Finally, many of the modules also have missing or empty AUTHORS files.


These issues were discovered while trying to construct debian/copyright
files.
