These are all separate issues that would need to be tracked separately, but

At the very least the kernel output, but preferably also the Xorg.0.log file
would be needed to diagnose this.

This could be a number of things, e.g. screensaver / DPMS / gamma (does running
xgamma -gamma 1.0 restore the normal display?) related.

Stability issues like that are usually AGP related, you could try playing with
all the possible values for Option "AGPMode" (see the radeon manpage) or even
disabling AGP altogether with Option "BusType" "PCI".
