I tried modes 1, 2, 4, 8 with an otherwise 'default' configuration as 
created by dpkg-reconfigure. 

Mode 1 is working for me.

You can find tested configuration files and results (server logs) in the 
attached tar file. There is a README which explains the contents a bit more. 
Sorry for the confusion and thanks again for your patience.
