cleaned up patch

A cleaned up implementation. I was unable to test the bicubic portion as it
does not work on RS690. :/

Other outstanding issues:

- The odd offset for scissor coordinates
- What to do about R200. The naming of the scissor registers imply that it
isn't available on R200. Is that so? Should we have a third code path for that?
