Tv-out isn't support yet on newer cards.  Also if your monitor is off
or disconnected there is no way for the driver to know if anything is
connected although in theory load detection should work.  Your config
looks fine.  what xserver are you using?  I looks like the core randr
code in the server is failing to force the output on.
