does it work if you start X without the external monitor attached? Is the
external port VGA or DVI? Does the ibook have a tv-out port and if so do you
have it connected?  If not, lets try hardcoding the outputs:

Add:

Option "ConnectorTable" "2,2,2,1,3,0,2"

to the device section of your config.
