[PATCH xserver] dix: Move {Change, Copy, Destroy}Clip from GCFuncs to Screen

Patch sent. Xnest and Xdmx took some special handling, but not
much. Everything else was just deleting bits. I took the opportunity to
change the GCFuncs initializers to use the field names instead of
relying on struct ordering.
