Mostly nuking dead or deprecated code.

The unifdef-XFIXES patch is a semantic change for DMX, which will now expose
the extension, but I think that's mostly fine.  The only part of Fixes I'd be
worried wouldn't work correctly under DMX is pointer barriers, which are
something of a convenience feature anyway.  At any rate they can't be more
broken than they are in Xnest, which does already expose the extension.
