[PATCH 2/3 v2] Introduce per-object per-screen privates.

This replaces dixCreatePrivateKey and the only uses, which were in
midispcur.

v2: Keith pointed out, in his usual manner, that having privates
behavior change depending on Xinerama didn't make any sense. Fixed.
