I had to upgrade my DocBook XSL stylesheets to 1.70.0 or later (I had 1.69.1)
and then fop 1.0 automatically produced them for the sections in the table of
contents.   (The bookmarks are the outline in the left column in Adobe Acrobat
for quickly jumping to a specific section.)

After that, all the docs I checked looked fine, so I added a note about that
to the commit log message and pushed.    Thanks to you & Matt for figuring
this out!  It should make navigation around the large docs like the X11 protocol
and libX11 specs easier.
