    doc: use xorg stylesheet and standard makefile
    
    The recent conversion to DocBook XML has created an infra-structure
    to apply a common stylesheet and build support.

    doc: use HAVE_XMLTO_TEXT for users without a text browser.
    
    Used to conditionally generate text documentation.
    xmlto for text output requires either lynx, links, or w3m browsers
