Log message:
  	- Use xf86RemoveEnabledDevice() in the DEVICE_OFF case too.
  	  Fixes a lock-up after server reset.
  	- Call InitPtrFeedbackClassDeviceStruct() during DEVICE_INIT
  	  phase, in order to avoid a segfault in the X server caused
  	  by 'xset q'.
